Library Card Application

All residents of Ohio, age five years or older, may obtain free registration for borrowing library materials. Patrons under 14 years of age must have a parent or guardian sign the application. A picture ID and a document showing current residence are required at the time of registration. Patrons may borrow up to 15 library materials at a time. A maximum of 4 DVDs may be borrowed at a time.

Other Borrowing Policies

Patrons may renew library materials online, over the phone or in person one time. Materials can be renewed additional times if not on reserve. Renewals are not permitted on DVDs.

Movies with an "R" or "TV/MA" rating will be limited to those 18 years or older.

For your protection and ours, you must present your library card for service, whether borrowing items or using the computers. Further, it is the policy of the library that no patron is permitted to borrow items on another patron's account. The only exception to this policy: materials for youth under 5 years of age are placed on the parent/guardian's account.

Borrowing and computer use privileges will be suspended when fines on an account reach $5.00.
